DIXIE ELECTRIC POWER ASSOCIATION
Laurel, Mississippi
APPLICATION FOR MEMBERSHIP AND FOR ELECTRIC SERVICE
The undersigned applicant, including spouse, if applicable (hereinafter singularly or collectively, called the “Applicant”), hereby applies for membership in and agrees to purchase and pay for electric energy from Dixie Electric Power Association (hereinafter called the “Association”) upon the following terms and conditions:
- The Applicant herewith pays to the Association the sum of $50.00, which, if this application is approved by the Board of Directors of the Association, shall constitute the Applicant’s membership fee.
- The Applicant shall, when it is available, purchase from the Association all electric energy purchased for use on the premises described below, and shall timely pay therefore monthly at rates which will be fixed by the Board of Directors of the Association. Any electric energy interconnected with the Association’s facilities shall be subject to rules and regulations fixed by the Board of Directors. The Applicant agrees to having said electric energy disconnected by the Association for failing to pay all amounts due according to policy, rules and regulations fixed by the Board of Directors. The Applicant and spouse if applicable, each agrees to be legally liable for the payment of all electric energy used. If the account becomes delinquent, applicant agrees to pay the balance due plus reasonable attorney and/or collector’s fees, court cost, and related expenses.
- The Applicant shall cause the premises to be wired in accordance with wiring specifications approved by the Association and agrees to pay a service charge of (a) $15.00 plus applicable State and Federal Tax between the hours of 8:00 a.m. and 4:00 p.m. (b) $40.00 plus applicable State and Federal tax between the hours of 4:00 p.m. and 8:00 a.m., or on weekends or holidays.
- The Applicant hereby agrees that part of the amount paid for electric energy shall be used to pay for a year’s subscription to the Today in Mississippi.
- The Applicant shall comply with and be bound by the rules and regulations of the Mississippi Public Service Commission, on the provision of the Charter and Bylaws of the Association, and such rules and regulations as may, from time to time be adopted by its Board of Directors.
- The acceptance of this application by the Board of Directors of the Association shall constitute an agreement between the Applicant and the Association to furnish electric service.
- The Applicant hereby grants an easement, right-of-way and the right of ingress and egress upon their property to the Association, and its agents and employees, for the purpose of performing all those acts necessary in construction, operating, maintaining and repairing electric service and equipment, including the clearing and maintaining of right-of way of the following widths: Ten (10) feet for insulated service and underground wire; Twenty (20) feet for bare service wire; Thirty (30) feet for single phase primary wire and forty (40) feet for multiphase primary wire. The Association shall have the right to widen said right-of-way to such widths when upgrading electrical service.
- As recipients of Federal Assistance, the Federal Government requires that you receive a copy of the Bylaws, Statement of Nondiscrimination and Rules and Regulation. Therefore, we ask that you please take a copy of each one with you so that you will be informed about your Association.
